I authorize ICON to request and obtain my child's educational records from their current and/or previous schools or educational programs. These records may include, but are not limited to, transcripts, report cards, attendance records, disciplinary records, standardized testing results, Individualized Education Programs (IEPs), Section 504 Plans, evaluation reports, behavior intervention plans, and other records relevant to my child's educational history and support needs.
I understand that this authorization is required as part of the admissions process and allows ICON to gather information necessary to evaluate my child's application and, if admitted, to appropriately plan for their educational needs. I certify that I am the child's parent or legal guardian and have the authority to provide this authorization.
